Delete your account in the app
The fastest way to delete your account is from inside the Ultradoc app. Deletion is immediate and permanent.

Open the Ultradoc app and sign in.

Go to Settings.

Tap Delete account.

Enter your password to confirm your identity.

Tap Delete my account and confirm. Deletion takes effect immediately.
Removing the app from your phone does not delete your account. Use the steps above, or Option 2 below, to delete it.

Request deletion without the app
If you can no longer access the app or your device, emailprivacy@ultradoc.netwith the subject line "Account deletion request", sent from the email address on your Ultradoc account.
We will verify that the request comes from the account owner and complete the deletion within thirty (30) days. We will confirm by email when it is done.

Your medical records. These are maintained by your healthcare practice — not by your Ultradoc account — and practices are required by law to retain them. Deleting your Ultradoc account does not alter or delete the medical record your practice holds.
Audit logs. Access and security logs are retained for the period required by HIPAA and our compliance obligations (up to six years), and are not associated with an active account after deletion.
Records we are legally required to keep, such as billing and payment records, for the retention period required by applicable law.
Questions about your medical record itself — including requests to access, amend, or restrict it — go to your healthcare practice, which remains the custodian of your record under HIPAA. Details are in ourPrivacy Policy.
