Delete your account

The fastest way to delete your account is from inside the Ultradoc app. Deletion is immediate and permanent.

   Open the Ultradoc app and sign in.

   Go to Settings.

   Tap Delete account.

   Enter your password to confirm your identity.

   Tap Delete my account and confirm. Deletion takes effect immediately.

Removing the app from your phone does not delete your account. Use the steps above, or Option 2 below, to delete it.

If the app stops you. Deletion is blocked in two cases, because deleting the account would strand someone else's care or a practice's records:

1. (i)You are the only parent or guardian on a child's profile that is connected to a practice. Deleting your account would leave that child's care with no one to manage it, so the app asks you to invite another parent or guardian first — or to use Option 2 below, where we handle it with you.
2. (ii)Your account also has staff or administrative access to a practice, or to Ultradoc. Those accounts are offboarded by the practice administrator rather than deleted from the patient app.

Email privacy@ultradoc.net with the subject line "Account deletion request", sent from the email address on your Ultradoc account. Use this route if you no longer have the app or the device, if you have forgotten your password, or if the app blocked deletion for either reason above.

Please include:

1. (i)The full name and date of birth on the account.
2. (ii)The email address and phone number on the account.
3. (iii)The practice or practices you are connected to, if you remember them.
4. (iv)Whether you also want any family profiles you manage to be deleted, and the name of each.

Or send a pre-filled request: open an email with these details.

What happens next. We acknowledge your request within five (5) business days and verify that it comes from the account owner (or, for a managed profile, the guardian who holds it) — we may ask you to confirm details already on the account. Deletion is completed within thirty (30) days of that verification, and we confirm by email when it is done. There is no charge for any of this.

If you are the sole guardian of a child's profile connected to a practice, tell us in the email how you would like that profile handled: transferred to another parent or guardian you name, or closed along with your account. Either way, the practice keeps the child's medical record, as described below.

1. (a)Your Ultradoc account and sign-in credentials.
2. (b)Your patient profile in the app (name, date of birth, phone number, and contact details).
3. (c)Your connections to practices inside the app.
4. (d)Push-notification registrations for your devices.

1. a.Your medical records. These are maintained by your healthcare practice — not by your Ultradoc account — and practices are required by law to retain them. Deleting your Ultradoc account does not alter or delete the medical record your practice holds.
2. b.Audit logs. Access and security logs are retained for the period required by HIPAA and our compliance obligations (up to six years), and are not associated with an active account after deletion.
3. c.Records we are legally required to keep, such as billing and payment records, for the retention period required by applicable law.
